Job Description:
This PRN Nurse Practitioner role involves conducting home risk
assessments for at-risk populations, including older adults and
community members. New grads are encouraged to apply, with
opportunities for flexible scheduling, including days, evenings,
nights, and weekends. Assessments, lasting up to one hour, cover
health history, social factors, medication adherence, and risks to
improve care quality and close care gaps. Responsibilities include
adult/geriatric evaluations, collaborating with healthcare teams,
providing health education, and documenting according to protocols.
Requires a Master's degree or equivalent, current RN and NP
licensure, board certification, BLS/ACLS, and at least one year of
NP experience. Travel is essential to meet patients at their
